The undersigned (“Customer”) purchased Vetericyn’s Products and used them on the animals that were photographed by the Customer. Vetericyn, Inc. desires to use the photographs and/or testimonials in various media as examples of its products and services.

Customer hereby consents to such use by Vetericyn, Inc. and its successors or assigns of the Photographs, slides and photographic images of the Products, and/or testimonials regarding effectiveness of Products, taken at Customer’s facility, by Vetericyn, Inc.
